this is a topic close to my heart, considering i, myself, work for an american manufacturer known for innovative and high quality products!
a few of my favorites:
esther williams swimwear
w.s. badger (natural personal care products, gilsum, nh; their sunblock is one of my favorite and one of the safest per EWG!)
indigo wild / zum (natural personal care products, kansas city, mo; best lip balms and soaps!)
sara's shoes (custom handmade shoes and boots, chicago, il; sara very well may be the closest tom bihn equivalent for footwear)
Another site that specializes in American Made products is keepamerica.com. They currently have a contest where they are going to give away a Ford Mustang to someone who sends in a picture of something made here. Also, they are giving 100% of their profits to cancer research (through July 4th).

they have a lot of good things going on even in the smallest details. for example, the strand of burlap that ties each bar's packaging shut is from a burlap sack that cacao was shipped in, which they pay folk attending a job training program to dismantle and tie onto each bar (training is done by springfield victory mission, which is one of the bigger homeless shelters in SW missouri; they do incredible, incredible work). those wages then are used to take field trips and contribute toward other programs benefitting fellow shelter residents.
when i'm in springfield on business (or pleasure; i used to live there and still visit from time to time), i always make a point to pick up a couple bars to share. especially this one: Askinosie Chocolate - Dark Chocolate + Malted Milk CollaBARation
